A school may apply here for one of two affiliation categories from this application (Subscriber School or Candidate Member). The appropriate category for the school will be determined upon the school's application material, an on-site assessment of the school by the NWAIS Executive Director, discussions with school leadership about the school's stage of development and the school's commitment to meet the obligations of NWAIS membership.  The NWAIS application fee is $200.

Please upload the materials requested below in the fields provided.  All supporting materials should be submitted to NWAIS prior to scheduling a school visit.

Application payment should be submitted to the NWAIS office at least one week prior to the scheduled school visit by the NWAIS Executive Director.  Your school's application fee of $200 can be submitted via Visa, Mastercard or check.
